Immunization Requirements for 2009-2010
Required forms for medication administration at school:
1. All prescription and non-prescription medications (except inhalers)
2. Self-Administration of asthma inhalers
3. Food Allergy Action Plan (or other life-threatening allergy): form #1 must also be completed.
